[silva-dev] [issue540] Unicode error if index item in heading contains special character

Silva 091 throws our friend the unicode error if an index item in a heading
contains a special character. E.g.:
[[Doré|Doré]]

To be specific [[Dor|Doré]] doesn't throw an error, so it goes wrong on the text
that appears in the document.

Haven't tried to reproduce this in Silva 092-3, but I assume the problem is the
same.
